LHC adds telehealth for care transitions, disease management

Cardiocom, the patient monitoring business unit of Medtronic plc has signed a partnership agreement with LHC Group, a national provider of hospice and post-acute care services. Under the agreement Medtronic will provide integrated clinical telehealth devices and other medical technologies to bolster LHC Group's care transitions and disease management programs.

By using Medtronic's Commander Flex remote patient monitoring devices, LHC caregivers will be able to conduct daily monitoring of symptoms and vital signs as a way to anticipate events and to intervene before the patient needs to be hospitalized.

"As more health systems and Accountable Care Organizations look toward maximizing value and population health models, post-acute care providers are playing a critical role in ensuring patients remain healthy through the transition from the hospital to their homes," said Daniel L. Cosentino, vice president and general manager of Medtronic's Cardiocom business, in a press release.
