PointClickCare announced today the availability of its change in condition evaluation solution, which is being demonstrated at booth 1101 in the exhibit hall at the American Health Care Association/National Center for Assisted Living annual meeting.
The company worked with the INTERACT team at Florida Atlantic University to blend the care management tools from the INTERACT 3.0 (Interventions to Reduce Acute Care Transfers) toolset—the SBAR (situation, background, assessment, recommendation) communication tool, decision support care paths and care cards—into one decision support tool, called eINTERACT. Integrated within the company's electronic health record platform, eINTERACT is designed to evaluate residents' conditions and determine the best approach for managing and communicating changes and alerts to the care team in real time.
“Combining these three tools not only simplifies the process for evaluating the resident appropriately; it also enforces the use of all of the INTERACT 3.0 decision support tools, which have been shown to reduce the readmission rates for our senior home customers,” says Dave Wessinger, chief technology officer of PointClickCare. Each hospital readmission costs about 33 percent more than the initial course of treatment for a resident, he adds, so preventing unnecessary transfers to hospitals and managing residents' conditions appropriately in-house is becoming even more important for care providers.
The eINTERACT change in condition evaluation tool is the only certified offering of its kind available in the market, according to PointClickCare.
