Common Formats for Nursing Homes Available for Review
The Agency for Healthcare Research and Quality (AHRQ) has released Common Formats for skilled nursing facilities, currently available as a beta version for public review and comment. AHRQ had contracted with the National Quality Forum NQF to assist with gathering and analyzing feedback on these formats.
The common formats include event descriptions, sample reports, and forms for both generic and event-specific categories. “Generic Common Formats,” which pertain to all patient safety concerns, are the Healthcare Event Reporting Form (HERF), Patient Information Form (PIF), and Summary of Initial Report (SIR).
“Event-specific Common Formats” pertain to frequently occurring and/or serious patient safety events and add specific, objective detail regarding these event categories. The event specific categories for skilled nursing facilities in the beta version are Device or Supply, including Health Information Technology, Fall, Healthcare-associated Infection, Pressure Ulcer, and Medication or other Substance.
