LTPAC, Senior Living and ACOs: Partnership Requirements and Strategies


Long-term care and senior living providers will have many opportunities to participate in ACOs and HIEs. To leverage these opportunities to the fullest and negotiate agreements that deliver the maximum benefit, long-term care providers will need well-developed strategies and health information technology systems that support them.  Acute care partners will be looking for quantifiable proof of the quality of care.
