Caremerge launches chronic care coordination solution

Chicago-based Caremerge has announced the launch of a chronic care management (CCM) solution aimed at coordinating chronic care management under Medicare’s new reimbursement coding system.

Since becoming part of the Affordable Care Act, Medicare’s reimbursement system focuses on wellness across the entire continuum of care and reimburses physicians for implementing aspects of care coordination that not only improve care but also reduce costs to the patient/resident.  

As a provider of cloud-based care coordination solutions for the senior care industry, Caremerge has included in its CCM solution an easy-to-use tool for physicians so they can coordinate and manage care for those with chronic illnesses in-house and in a streamlined manner that eliminates having to pay outside providers.
