LeadingAge CAST launches care coordination technology selection tools
The LeadingAge Center for Aging Services Technologies (CAST) has released a new set of product comparison tools to help providers choose the best care coordination and care planning software for their needs. The portfolio includes an online product matrix, whitepaper guides on the issues relating to care coordination technology and five case studies.
Care coordination and shared treatment planning includes everything from team-based information at the provider site to disease management education for patients at home. The whitepaper and guides help providers understand the elements of care coordination technology and the potential applications for improved outcomes.
“Shared care planning and coordination technologies can help providers improve communication, quality of care, patient engagement and satisfaction,” said Majd Alwan, PhD, executive director of CAST and LeadingAge senior vice president for technology.
CAST offers product comparison and selection tools for several technology categories, including electronic health record systems, medication management systems and telehealth.
