Brookdale tackles advance care planning
Brookdale Senior Living has partnered with Iris Plans to offer telehealth advance care planning (ACP) for residents.
Iris Plans will offer videoconference discussions with its facilitators to residents, allowing them to create a plan that guides important and complex healthcare decisions and share them to anyone they choose, including families, doctors and hospitals.
“Advance Care Planning through Iris Plans is a great compliment to the many patient-centered services we offer at Brookdale Senior Living,” said Todd Mackenzie, Executive Director and Austin Market Leader for Brookdale Senior Living. “Iris Plans will further enable Brookdale residents to connect and with their families and loved ones, and will help our residents plan for healthcare decisions, reduce stress around these decisions, and help avoid difficult and unnecessary hospital stays.”
