Qualcomm launches chronic care management app

San Diego-based Qualcomm Life Inc. announced the launch of a new mobile app for iOS and Android devices to help seniors manage their chronic conditions more effectively.

The HealthyCircles Care Coordination platform provides access to vital health information and a secure communications platform for care team coordination and is built on Qualcomm’s Life 2net mobile core which seamlessly captures biometric data from medical devices.

“As care is moving from the hospital to the home, it’s crucial that patients have an easy way to track their health and communicate with a care team,” said Rick Valencia, senior vice president and general manager of Qualcomm Life, in a press release.

James Mault, MD, FACS, vice president and chief medical officer at Qualcomm Life, added: “HealthCircles Mobile is an adaptive, personalized mobile care experience for patients who need it the most: people with chronic conditions and those transitioning from hospital to the home. Our goal is to provide a frictionless platform that will guide healthy behaviors and motivate patients to engage in their own self-care.”
