NTOCC launches care transitions resource database

The National Transitions of Care Coalition (NTOCC) has launched its Knowledge and Resource Center, an active database of collaborative tools and educational materials for improving care transitions.

The online center will serve as a collection of resources and technology submitted by long-term care providers, acute care providers and application developers based on evidence-based care interventions. Accepted tools must fulfill core missions in improving transitions of care and must support at least one of NTOCC’s seven essential interventions, which include medication management, transition planning, patient/family engagement, information/data transfer, follow-up care, provider engagement and shared accountability.

The first two resources in the center are Care4Today Mobile Health Manager and Janssen Connect, two medication management applications from the Janssen Pharmaceutical Companies. Other tools and materials will follow in the near future, according to an NTOCC press release.

NTOCC also offers a library of whitepapers, case studies and journal articles on care strategies, resident engagement, follow-up care and other aspects related to improving care transitions and quality outcomes.
