New partnership will help personalize LTC care

A partnership between Chicago-based Caremerge LLC and Washington, D.C.-based Linked Senior Inc. will engage seniors in their own wellness and help the long-term care staff personalize resident care.

The partnership between Caremerge, a company that provides a cloud-based coordination and communication platform for the healthcare team, seniors and their families; and Linked Senior, a provider of engagement tools for the LTC market, will integrate each other's technologies to provide an interactive, resident-centered engagement solution that will track physical and cognitive abilities.

According to a press release posted on Linked Senior's website, information and data collected from Caremerge’s technology solution will enable Linked Senior to assist staff in building customized programs in each community to engage seniors from a life enrichment and wellness perspective. Conversely, physical and cognitive information about seniors from Linked Senior's solution will allow Caremerge to identify and develop effective care plans for seniors by matching such information with other social and clinical data already residing within Caremerge.
