Medalogix partners with Homecare Homebase

Nashville, Tenn.-based analytics company Medalogix has partnered with Dallas-based Homecare Homebase LLC to provide Homecare Homebase clients the ability to accurately identify those who are most at risk of transferring off census, or who would benefit from an additional care episode or hospice care.

Homecare Homebase, which serves the post-acute care industry through home healthcare, hospice and private duty, offers an integrated, web-based software solution that enables real-time information exchange between office staff, field staff and physicians working with homecare and hospice agencies. By teaming up with Medalogix, the company will now have access to Medalogix's two analytics solutions: Touch, which helps to reduce transfers off census and to identify patients in need of additional care episodes, and Bridge, which helps identify and inform patients who would benefit from hospice care.
