The interface of long-term care and home health will be the topic when Long-Term Living co-hosts a Twitter chat with the Alliance for Home Health Quality and Innovation (AHHQI) on Tuesday, Aug. 26, at 2 p.m. ET.
To participate in the conversation, follow the hashtag #HomeHealthChat, and use that hashtag in all of your tweets. Following the conventions of Twitter, all tweets are limited to 140 characters, including the hashtag.
The forum will be discussing the following questions and more (participants are encouraged to think about these questions and formulate answers in advance):
Q1: What should home health agencies know about skilled nursing & assisted living communities, and vice versa?
Q2: How can home health partner with SNFs and ALFs in the delivery of high-quality care?
Q3: What are some of the things home health and SNFs/ALFs have in common?
Q4: What challenges exist for home health, SNFs and ALFs?
Q5: How can partnerships between home health and SNFs or ALFs help prevent avoidable hospital readmissions?
Q6: What emerging models use home health and at least one of these communities, and how can they shape the future?
Q7: What resources support partnerships between home health and SNFs or ALFs?
The AHHQI hosts Twitter chats on the third Tuesday of every month. For more information about this Twitter chat, to view a "how to" video designed to aid in your participation and to read recaps of previous events, visit this page on the organization's website.
